Case Managers can help older adults and adults with disabilities who need personal care assistance to access the services and resources they need. Services include:
A visit to your home to assess your needs and develop a care plan
Referral for other community-based services as needed
Arrangement for you to receive supportive services such as home-delivered meals or transportation to medical appointments
Regular contact with you to ensure that the services you are receiving continue to meet your needs
Options Counseling is a free service provided though the ADRC to all seniors and people with disabilities (18 and over), or their family members, to help them plan for their long-term care needs. These services are provided by telephone or in person, which includes home visits. If you would like to speak with an Options Counselor, please call 541-682-3353.
